The Questionnaire collects :

both personal and personnel information, some fields may be customised for management reporting
working conditions, type of work, hours and break patterns,
perceived problems, Visual, Environmental and General.

The program can be modified to add or omit various questions.

Information collected forms a fundamental part for the analysis of the vision tests and reports, however, for special cases it may be omitted.
